Fuchs虹膜异色性葡萄膜炎20例临床分析

Clinical Analysis of Fuchs Heterochronic Uveitis 20 Cases

目的探讨Fuchs虹膜异色性葡萄膜炎的临床特征及临床诊治中的常见误区。方法对20例Fuchs虹膜异色性葡萄膜炎患者的临床资料进行回顾性分析研究。结果20例患者中19例为慢性前葡萄膜炎,1例为慢性全葡萄膜炎;单眼患病者17例;全部23只患眼均有特征性KP,17眼有虹膜异色。12眼并发白内障,6眼继发青光眼。结论Fuchs虹膜异色性葡萄膜炎,发病隐匿,炎症轻微;常见的体征有特征性KP、虹膜异色等;主要并发症为并发性白内障和继发性青光眼;此病误诊现象多见,糖皮质激素滥用是目前此病临床治疗中最常见的误区。

Objective To investigate the clinical features of Fuchs heterochronic uveitis and the common errors in its diagnosis and treatment. Methods A retrospective study was performed on the clinical data of the 20 patients with Fuchs heterochromic uveitis. Results Among 20 patients,19 cases presented as chronic anterior uveitis and 1 case as chronic panuveitis. 17 patients had only one eye affected. Characteristic keratic precipitates(KP) was seen in all of the 23 suffered eyes, heterochromia was established in 17 eyes, 12 eyes complicated with cataract and 6 eyes with secondary glaucoma. Conclusion Fuchs heterochronic uveitis most cases are asymptomatic with only mild inflammation. The usual clinical manifestations conclude the characteristic KP and heterochromia, etc. The main complications are complicated cataract and secondary glaucoma. Misdiagnosis of this disease could be seen commonly in the current clinical practice. The usual error in its treatment was abuse of glucocorticoid.
