老年病人全麻复合硬膜外阻滞对上腹部手术呼吸功能的影响

The effects in pulmonary function of upper abdominal surgery under generalanesthesia combined with epidural block anesthesia in aged patients

目的评价不同的麻醉方法对老年病人呼吸功能的变化及其影响。方法选择30例呼吸功能正常的老年上腹部手术病人,随机分为两组,实验组(Ⅰ)为全麻复合硬膜外阻滞;对照组(Ⅱ)为单纯全麻。分别在术前3d、术后1d、术后3d,监测最大自主通气量(MVV)、分钟通气量(VE)、一秒用力吸气量(FEV1)、残气量(RV)、总肺容量TLC、动脉二氧化碳分压(PaCO2),动脉氧分压(PaO2)的变化。结果实验组呼吸功能指标比较无明显差异;对照组呼吸功能指标比较有明显差异。结论全麻复合硬膜外阻滞对老年病人呼吸功能影响小,是老年病人比较理想的麻醉方法。

Objectives To evaluate the changes in pu lm onary function in old patient of differents anesthesia.methods Thirty elderly patients scheduled for elective abdominal operation were randomly di vided into two groups (experimental group,group Ⅰ) and general anesthesia group (control group,group Ⅱ).Maximal voluntary ventilation(MVV),minute volume of d ead space ventilation(VE),forced expiratory volume in one second(FEV1),residua l volume(RV),total lung capacity(TLC),arterial partial pressure of carbon diox ide(PaCO2),arterial partial pressure of oxygen(PaO2) were recorded repectiv ely 3 day before operation and 1 day and 3 day after operation.Result The difference was not significant among pulmonary function test of the gr oup Ⅰ.but were significant among the groupⅡ.Conclusion Gaeneral anesthesia combined with epidural block anesthesia dose not affect respiratory function,and it is a good anesthesia to aged patients.
