非开胸食管内翻拔脱术治疗颈段及上胸段食管癌(附45例报告)

Inversion-stripping Esophagectomy without Thoracotomy in the Treatment of Cervical and Upper Thoracic Esophageal Carcinoma

本文报告了我科非开胸食管内翻拔脱术治疗颈段及上胸段食管癌45例.其中颈段食管癌22例,上胸段23例;病变位于颈段者采用左侧颈部切口,位于上胸段者行颈胸骨部分劈开切口.术后发生颈部吻合口瘘7例,声嘶5例,全组元手术死亡.文中还就手术方法、体会及吻合口瘘的预防进行了讨论.

This paper reports the results of inversion-stripping without tho-racotomy in the treatment of 45 patients with carcinoma in cervical and upper thoracic segments of esophagus. The left cervical incision was perfoimed for cervical esophageal carcinoma and partial cervicosternotomy was performed for lesion in the upper thoracic segment. The postoperative complications such as cervical anasto-motic fistula and hoarseness occurred in 7 and 5 cases respectively. There was no operative death in this series. The operative technic, experience and prevention of the anastomotic fistula were discussed in this paper.
