心房起搏法在房室结射频消融术中的应用

摘要 目的探讨对房室结折返性心动过速,射频消融术中应用心房起搏的效果。方法6l例房室结折返性心动过速被随机分为心房起搏组(n=30)、常规组(n=31)进行射频消融术,比较两组消融放电次数、时间、能量及并发症、复发率等。结果与常规组比较,心房起搏组缩短总放电时间(132.6±48.2svs178.4±58.4sP<0.05),增加单次持续放电的成功率(93.3%vs6.5%,P<0.001),同时可减少术中并发症和降低术后复发率,常规组有3例术中出现一过性房室阻滞,1例永久性房室阻滞,3例术后1年内复发,而起搏组无一例出现上述现象。结论房室结射频消融术中采用心房起搏法进行放电比常规法更为安全有效。 Objective To explore the effect of atrial pacing using in the procedure of radiofrequency atrioventricular node modifiaction. Methods Thirty cases who received atrioventricular node modification under atrial pacing were compared with another thirty-one cases who received the procedure with routine method. Results The method with atrial pacing was effective in reducing the total discharge time (132.6±48.2s vs 178,4±58.4s P<0.05);increasing the success rate by single discharge(93.3% vs 6.5% P<0.001)and decreasing the complication and recurrence rate. There were 3 cases occurring I degree atrioventricular block temporarity while discharge;1 case occurring permanent complete atrioventricuiar block and 3 cases recurred during one year after the procedure in rontine group,but no one of those in pacing group. Conclusion Atrial pacing method is more safe and effective in the procedure of radiofrequency atrioventricular node modification [
