小于胎龄儿脐血T_3、T_4等六种激素水平测定 被引量:5

摘要 测定17例早产和125例足月小于胎龄(SGA)儿脐血三碘甲腺原氨酸、甲状腺素、促甲状腺素、胰岛素、皮质醇和生长激素水平,并与原测定的64例早产和308例足月适于胎龄(AGA)儿的数值相比较。结果:(1)SGA儿的甲状腺素分泌受抑制,足月儿比早产儿明显,出生体重在第3百分位数(k)以下的重度SGA儿比第10百分位数(P_(10))以下的SGA儿明显,表现为低T_3综合征,说明较重的早产SGA(P_3)儿或足月的SGA儿甲状腺功能受到一定的抑制,这可能与在宫内经受慢性缺氧和酸血症较重,或时间较长有关;(2)SGA儿胰岛素水平随胎龄增加而下降,且SGA(P_3)比SGA(P_10))下降更甚,可能与在宫内时间愈长、SGA儿愈重,受到慢性缺氧和酸血症影响愈大有关;(3)SGA儿的皮质醇水平随胎龄增加而升高,生长激素水平SGA儿较AGA儿为高。 AbstractTriiodothyronine (T_3), thyroxine (T_4), thytoitistimulating honnone, insulin, cortisol and humangrowth honnone concentrations were ineasubo incord serum of small for gestational age (SGA) infants,consisting of 17 preteim (GA 34<37wks) and 125terni (GA37 <42wks), and were comparorl with thosemesured in 64 pretenn and 308 tenn appropriate forgestational age (AGA) infants in another study. It wasfound that (1) the thyroid hormone secretion in SGAinfante seemed to have ben suppresed to a certaindegrec, showing the characteristics of . Low T3syndrome' ; (2)insulin levels in cord senini of SGA in-fants decreased as gestational age advantal, suggesting that chronic hypoxia and acidosis had some effects on insulin secretion; (3) coneentrations of humangrowth hormone in cord seam of SGA infants weresignificantly higher than those of AGA infants, forwhich the mechanisms potentially involoved are in-creased pituitary responsiveness to GH-releasinghornione and decreased inhibitory feedback by cireu-lating insulin-like growth factor-1 (IGF- 1)
