喉近全切除术──Pearson手术在晚期喉癌及下咽癌治疗中的应用

Ncar-total Laryngectomy in the Treatinent of Advanced Laryngeal and Hypo-pharyngeal Carcinoma

利用Ｐｅａｒｓｏｎ术式治疗１４例患者，其中喉癌１０例，梨状窝癌３例，下咽颈段食管癌１例。除２例发育管闭锁外，其余１２例均在术后２～５周内获得理想的发音和良好的无呛咳吞咽功能。２年生存率７８．６％，详细介绍了手术操作步骤及若干改进并扩大了的手术适应症。

Near-total laryngectomy was first introduced by RB Pearson in 1 980.In this report,14cases(T_3,T_4 laryngeal cancer 1 0 cases,pyriform fossa 3,cervical esophagus 1 )treated with near-total laryngectomy .The speech function was obtained in 1 2 / 14(85.7%)within 2 to 5 weeks, all cases hadgood swallowing.2-year survival rate is 78.6%. The authors believed that such surgical technic is most indicated in the cases when regular partial laryngectomy can not be performed and in elderly cases who would have the risk with partial laryngectomy.
