鼻咽癌p16和nm23-H_1基因表达的意义 被引量:3

The relationship between p16 and nm23 expression in the tissues of nasopharyngeal carcinoma and T-AgNOR activity in peripheral blood of the cases

摘要 目的观察p16、nm23-H1基因在鼻咽癌(NPC)组织中的表达状态,探讨其与NPC转移及与外周血T淋巴细胞核仁形成区嗜银蛋白(AgNORs)水平的关系。方法采用S-P免疫组织化学方法检测65例NPC初诊患者癌组织中p16、nm23-H1,并检测治疗前患者外周血T-AgNORs含量。结果p16阳性表达率为24.6%(16/65),nm23-H1为61.5%(40/65)。其中,p16表达与T分期及有无颈淋巴结转移及放疗后远处转移无相关,(P>0.05);而nm23-H1蛋白在有颈淋巴结转移的患者中表达率为51.2%(21/41),低于无颈淋巴结转移者79.2%(19/24),P<0.05,随访中15例发生远处转移,其nm23-H1表达率为33.3%(5/15),显著低于无转移者的70.0%(35/50),P<0.01,但其表达水平与T分期无相关。p16、nm23-H1表达阳性患者外周血T-AgNORs含量显著高于阴性者,(P<0.01)。结论p16、nm23-H1基因的表达不仅可能涉及鼻咽癌的发生发展过程,而且可能与机体的免疫调节机能有关。 Objective To investigate the relationship between p16 and nm23-H_1 expression in the tissues of nasopharyngeal carcinoma(NPC) and T-AgNOR activity in the peripheral blood of cases with this tumor. Methods Sixty-five cases with NPC were included in this study as observing cases and other 31 healthy persons were taken as controls.The tissue samples collected from the nasopharynx of all the cases were assayed by immunohistochemical S-P procedures to determine the expressing levels of p16 and nm23-H_1 and the peripheral blood samples of them were tested by silver staining and image analyzing procedures to investigate the immune function of T cells among these cases before treatment.Then,analyzed was the relationship between the two kinds of indicators of them. Results The positive expressing rate of p16 was 24.6%(16/65) in the tissues of NPC,without significant association with T staging of the primary foci and metastasis to local neck lymph nodes or distal organs(P>0.05).In contrast,the positive rate of nm23-H_1 was 61.5%(40/65) in the tissues of NPC,with significantly higher positive rate in the cases with neck lymph nodes metastasis than that of cases without this condition(51.2%vs.79.2%,P<0.05).Furthermore,the positive expressing rate of nm23-H_1 was significantly higher among the cases with general metastasis(33.3%,5/15)than that of ones without this kind of metastasis(70.0%,35/50),as seen during the following up period(P<0.01),while the positive expressing level of nm23-H1 was seen with no significant association with T staging of the primary foci among these cases(P>0.05). On the other hand,the content of T-AgNORs in peripheral blood was significantly higher among the cases with positive expression of p16 and nm23-H_1 in tumor tissues than those with negative expression of these two indicators (P<0.01). Conclusions p16 and nm23-H_1 expression may be involved in the process of NPC developing and their expressing levels are associated with the activity of T-AgNORs in peripheral blood,which can be taken as one of the indicators to evaluate the immune function of cases with malignant tumors.
