不同起搏方式对病态窦房结综合征预后的影响

Influence of different pacing modales on the prognosis in patients with sick sinus syndrome

目的:比较单心室起搏(VVI)和双腔起搏(DDD)对病态窦房结综合征(SSS)患者快速房性心律失常和心功能的影响。方法:对97例VVI、20例DDD患者进行6个月随访,观察临床症状、心电图、24 h动态心电图、心脏超声等变化情况。结果:与DDD组相比,VVI组术后持续性心房纤颤及新出现快速房性心律失常的总发生率较高(P<0.05),左心房直径(LAD)增大、左心室射血分数(LVEF)下降较明显(P<0.05)。结论:SSS患者DDD的预后优于VVI。

Objective: To compare the influence of VVI and DDD pacing model on the incidence of atrial tachyrhythmia and heart function in patients of sick sinus syndrome (SSS). Methods : A 6 months follow-up survey including clinical examination, electrocardiography, dynamic electrocardiography and ech-ocardiography was carried out in VVI(97cases) and DDD(20 cases)pacing patients. Results:Compared with the DDD group, the total incidence of sustained artial fibrillation and new atrial tachyrhythmia post operation in VVI group was much higher, left atrial diameter(LAD)much larger, and the left ventricular ejection fraction ( LVEF) decreased more significantly. Conclusion:The prognosis of SSS in the DDD group is significantly superior to that in the VVI group.
