年长儿童先天性髋关节脱位患者，术前的骨牵引效果的好坏，将直接影响矫形手术的效果。而在影响骨牵引的诸多因素中，髂腰肌和内收肌挛缩是主要的。骨牵引前切断挛缩的髂腰肌和内收肌，然后行骨牵引，这就解除了这二组肌肉对骨牵引的干扰，使其骨牵引更好地去克服载荷应力改变的影响。取得了良好的牵引效果。缩短了牵引时间。为矫形手术打下好基础。

AbstractThe results of preoperative skeletal traction in elder chilren with congenital dislocation of hip (CDH)directly influence the outcome of surgical correction of CDH.Sec-tion of the contracted iliopsoas and adductor muscles elimi-mates the disturbance of the two muscle groups on skeletal traction.The period of skeletal traction can thus be short-ened and the foundation of surgical correction of CDH is laid. Thirty two cases so treated were reported in this pa-per.
