老年人腹腔镜胆囊切除术的围手术期处理

Perioperative managementof laparoscopic cholecystectomy in aged patients

目的探讨腹腔镜胆囊切除术治疗老年人胆囊良性病变的临床价值。方法:回顾性分析216例老年患者行腹腔镜胆囊切除术的围手术期处理结果。结果216例中,慢性结石性胆囊炎147例,急性胆囊炎26例,胆囊息肉32例,其他11例。中转开腹2例(0.92%),其余顺利完成LC,无胆管损伤,无严重并发症,无手术死亡。住院天数3～12天,平均7.5天。结论只要经充分术前准备,掌握手术适应证,把握手术时机及技巧,做好围手术期处理,老年人行腹镜胆囊切除术是安全可行的手术方法。

Objective:To estimate the value of laparoscopic cholecystectomy in the aged patients with benign lesions of the gallbladder.Methods The results of laparoscopic chdecystectomy in 216 aged patients were analysed retrospectively.Results Among the 216 cases,there were calcuous chocecystitis in 147cases,acute cholecystitis in 26 cases,and polyps in 32 cases,the other in 11 cases.2 cases were converted to an open cholecystectomy(0.92%).Other 214 patients had LC successfully without serious complication.3～12 days were cost in hospital,averaged 7.5 days.Conclusions Under a carefully prepare before operation,Strict indication and command operative time and techrical skill,laparoscopic cholecystectomy is a safe and feasible operation for aged.
