不典型急性硬膜外与硬膜下血肿的CT鉴别

CT features and differential diagnosis of atypical acute epidural hematoma and subdural hematoma

目的探讨颅脑损伤不典型急性硬膜外与硬膜下血肿的CT影像学特征及鉴别要点。方法回顾性分析CT影像学表现不典型的79例病例,与术中证实或后期CT复查及MRI等的确诊结果相比较、分析。结果首次CT平扫检查误诊率较高。结论不典型急性硬膜外与硬膜下血肿在CT影像学鉴别诊断上相对困难,需要较高的技巧,有时需要CT复查或MRI检查及相关临床资料辅助。

Objective: To discuss the atypical CT imaging characteristics of acute epidural hematoma and subdural hematoma in head injuries. Methods: The atypical CT imaging characteristics in 79 cases were analysed retrospectively, the results were compared with surgical operation, CT follow-up and MRI examination. Results: There is rather high erroneous diagnostic rate according to the first CT scan imaging. Conclusion: It is difficult to make differential diagnosis for atypical CT features of acute epidural hematoma and subdural hematoma, it relies on experienced technique, CT follow-up and MRI examination.
