腰椎小关节穿刺造影的初步研究

Arthrogrphy of the Lumbar Facet Joints

目的：分析腰椎小关节的多种造影表现，探讨其临床意义、资料和方法：选择腰腿痛患者35例，在X线电视监视下施行腰椎小关节穿刺造影及封闭治疗。结果：本方法成功率高达92％，未见并发症。可清晰显示小关节内部结构，特别是关节囊的形态改变，其中椎弓峡部不连、假性脊柱滑脱及小关节本身的某些病，变均伴有相应的改变。结论：本方法能准确、客观地反映小关节的多种形态改变，在腰腿痛的诊断及治疗中有较高价值。

Purpose: By analysing the arthrographic appearances of the facet joint,to investigate their clinicalsignificanccs. Materials and Methods: Fluoroscopically guided, intra - articu1ar facet arthrography and blockingwere performed in 35 selected cases with chronic low back pain and sciatica,inc1uding 73 articulations. Results: Ahigh success rate was obtained in the procedure of arthrography without complication in any case. The internalstructures of the facet joint as well as the morphologic appearances of the capsule were displayEd.Among them,spondylolys is, spondylol is thes is and s ome other arthropa thies were always correlated with s pecific arthrographicappearances. Conclusions:With lumbar facet arthrography most alterations in radiological appearance of the articulations can be presented both clearly and objectively. It is quite useful in evaluating low back pain and sciatica andthus deserves further research.
