高血压脑出血的手术治疗

Operative treatment of hypertensive intracerebral hemorrhage

目的分析189例高血压脑出血手术疗效并探讨提高手术疗效与生存质量的相关因素。方法回顾性分析了189例高血压脑出血病人手术治疗的临床资料,分析不同手术时机、手术方法的治疗效果。结果死亡21例。其余随访6月￣3年,平均1.5年,按照日常生活能力(ADL)量表来评定疗效:Ⅰ￣Ⅴ级分别为35.45%(67/189),22.22%(42/189),16.93%(32/189),7.94%(15/189),6.35%(12/189)。结论早期手术解除脑部受压是治疗的关键,采用微创手术治疗是提高生存质量的重要手段。

Objective To discuss the clinical effect of surgical treatment for 189 cases of hypertensive intracerebral hemorrhage,and investigate the relative factors of improving the curative effect and prognosis. Methods 189 cases was retrospectively analysed for finding the relationship among the surgical opportunity,methods and post-surgical rehabilitation. Results The death rate was 11.11%(21/189). The cases was followed up for about one year and a half. According to activities of daily living,grade Ⅰ~Ⅴ was 35.45%(67/189),22.22%(42/189),16.93%(32/189),7.94%(15/189),6.35%(12/189),respectively. Conclusion Early operation and decrease of intracerebral pressure is the key factor in the operation. Microsurgery and timely recovering treatment are important for the living quality later.
