An improved technique for bloodlese hepatic resectionon the in situ cold perfused liver

作者报告了一种改进的全肝血流隔离、原位低温灌注的无血切肝技术，以处理累及肝门区和主要肝静脉、肝后下腔静脉的复杂的巨大肝肿瘤。作者描述了该种技术不同于Ｆｏｒｔｎｅｒ方法的操作要点，报道了采用该种技术切除两例巨大小儿肝肿瘤，重建门静脉，修补肝静脉和下腔静脉破口的成功经验。

AbstractAn improved technique for bloodless hepatic re-section using in vivo isolation and asanguinous hy-pothermic perfusion was described to deal with thehuge liver tumour involving the liver hilum, the mainhepatic veins and retrohepaic inferior vena cava. Theoriginal Fortner’s technique was modified, includingthe incision;isolated cold perfusion of the healthy halfof the liver through the ported vein;suprahepatic out-let for the perfusate, and shortened hepatic ischemiatime by perfusing hepatic artery prior to the repair orreconstruction of the portal vein.Using this tech-nique,two huge liver tumors were successfully resect-ed, and the indications,intraoperative monitoring andearly postoperative courses were discussed
