取栓加溶栓联合治疗中期动脉栓塞

Treatment of middle stage arterial embolism throughboth Fogarty catheter embolectomy and directly pour-ing thrombolytic agents into artery

我们采用Ｆｏｇａｒｔｙ导管取栓加动脉内直接灌注溶栓药物治疗中期动脉栓塞９例，均取得成功，所有肢体均获挽救且症状消失。本组９例治疗距发病时间１５小时至４０天不等。作者认为取栓的适应证不仅取决于发病时间，更重要的是肢体有否发黑坏疽。采用取栓加溶栓联合疗法，不但能取除主要动脉内的栓子，还可通过动脉内直接灌注溶栓药物而溶解细小动脉内的继发血栓故效果较好。

AbstractWe treated successfully 9 patients with middlestage arterial embolism through both Fogarty catheterembolectomy and directly pouring thrombolytic agentsinto artery, In all patients the extremities were sal-vaged and symptoms disappeared,The patients wereoperated upon 15, 30, 60 hours and 3,5,8,30,39,40days respectively after onset of the disease. The indica-tions for this treatment are dependent on how manyhours last between onset and treatment and whetherthe extremity is gangrened. The main embolus in thegreater artery can be removed by Fogarty catheter,whi卜thethrombosis in smaller arterv can be resolvedby directly arterial luminal pouring thrombolytic a-gents , so the result is better if the two methods arecombined.
