Rh血型不合引起的输血反应

Haemolytic transfusion reaction of surgical patientsdue to incompatibility of Rh blood groups

作者报告４例因Ｒｈ血型不合引起的溶血性输血反应。其中１例患者反应发生早而重，因未能及时确诊，最终死于肾功能衰竭，其余３例为迟发性溶血反应，反应轻，经及时处理未造成严重损害。由于我国汉族人群中Ｒｂ血型不合少见，输血前未常规进行Ｒｈ血型交叉试验，加之此种溶血反应的临床表现与ＡＢＯ溶血截然不同，故早期易误诊。如不能将其与手术创面渗血引起的贫血鉴别，再次输入Ｒｈ血型不合血，将发生严重溶血。此种溶血应引起外科医生足够的重视。

AbstractHaemolytic transfusion reactions occured in 4 padents under plastic surgery due to incompatibility ofRh blood groups. The reaction in one of the patient oc-cured not only abrupt but severe and finally died of re-nal failure. Other three patients with delayedhaemolytic reactions survived after treatment. Sincemore than 99.5%Chinese population is Rh positive,Cross-matching on Rh blood groups is not a routine.The reactions develop usually different from typicalABO blood group haemolytic reactions and are not easyto make an early diagnose. If the surgical patientsshow profound anemia and haemorrhage followingtransfusion which could not be explained by bleedingand coagulation abnormalities, haemolytic transfusionreactions due to incompatibility of Rh blood groupsmight be considered.
