浙江两社区居民2型糖尿病患病率及危险因素研究

Study on The Morbidity of Type II Diabetes and Risk Factors among Residents from Two Communities of Zhejiang Province

目的 了解城乡社区人群2型糖尿病患病率及危险因素,为开展社区干预提供科学依据。方法 选择2个县区,对74 17名35岁及以上常住居民进行2型糖尿病流行病学调查。对现患病例采用成组病例对照方法,进行危险因素logistic回归分析。结果 2型糖尿病标化患病率为5 92 % ,随年龄增加而上升(χ2 =16 0 89,P <0 0 5 ) ,城市2型糖尿病标化患病率(5 89% )高于农村(5 2 2 % ) ,农村女性标化患病率(6 0 9% )高于男性(4 30 % )。糖尿病家族史、恶性肿瘤史、高血压史、超重、年龄为城市社区人群危险因素(OR值分别为3 99、1 96、1 89、1 0 6、1 0 4 ) ,糖尿病家族史、高血压史、海产品摄入多、肥胖、年龄为农村危险因素(OR值分别为2 33、1 80、1 2 1、1 11、1 0 3) ,水果摄入多为城乡社区人群保护因素。结论 控制体重、高血压,倡导合理膳食。

Objective This research was to learn about the morbidity of type II diabetes and related risk factors among community residents in towns and countries and provide scientific basis for the implementation of community intervention.Methods 2 counties were selected and epidemiological investigation on type II diabetes was conducted for 7 417 permanent residents aged 35 or more. As for current sufferers, case comparison was used and logistic regression of risk factors was managed.Results The standardized morbidity of type II diabetes was 5.92%, which increased along with age (χ 2=160.89, P<0.05); the standardized morbidity of type II diabetes in cities (5.89%) was higher than rural regions (5.22%); in rural regions, that of female (6.09%) was higher than male (4.30%). Familial history of diabetes, history of malignancy, history of hypertension, overweight and age were risk factors for community population in cities (OR was 3.99, 1.96, 1.89, 1.06, 1.04 respectively); Familial history of diabetes, history of hypertension, superfluous intake of sea food, obesity and age were risk factors for rural regions (OR was 2.33, 1.80, 1.21, 1.11, 1.03); high-level intake of fruits were protective factors for community population.Conclusion Control of body weight and hypertension, advocacy of rational meals and reinforcing physical exercises are important measures for community residents to guard against and control type II diabetes.
