扩张型心肌病抗心肌线粒体ADP／ATP载体自身抗体的研究

A study of the autoantibody against an ADP/ATP carrier of myocardial mitochondria in dilated cardiomyopathy

应用免疫转印技术和细胞毒试验检测２１例扩张型心肌病（ＤＣＭ）患者血清中抗猪心肌线粒体ＡＤＰ／ＡＴＰ载体抗体。结果发现，ＤＣＭ患者血清中可检测出抗心肌线粒体３０千道尔顿（ｋＤ）多肽（ＡＤＰ／ＡＴＰ载体）自身抗体，检出率为５２．４％，而１９例对照组血清均为阴性（Ｐ＜０．００１）；该抗体与猪心肌细胞膜５２ｋＤ多肽发生交叉反应，直接引起的心肌细胞毒损害具有钙依赖性，可通过钙拮抗剂预防。本研究提示，抗心肌线粒体ＡＤＰ／ＡＴＰ载体自身抗体可能是引起ＤＣＭ心肌功能损害的原因之一，检出该抗体可作为ＤＣＭ的重要诊断指标。

Abstract The antibodies against ADP/ATP carrier (ANT) of pig heart mitochondria were analysed in sera of patients (n=21) with dilated cardiomyopathy(DCM) by immunoblot technique and cytotoxicity test. The deteetable rate of the autoantibody against a 30 kD polypeptide (ADP/ATP carrier)of pig heart mitochondria in patients was 52.38%,and zero in controls (coronary heart disease, n = 7; rheumatic heart disease, n=7, and healthy volunteers, n= 5)(P <0.001), The autoantibody crossreacted with a 52 kD polypeptide of pig myocardiai cell memberane(MCM),and induced directly myocardial cytotoxic damage that was Ca2+ - dependent and might be pre vented by calcium channel blocker. The study suggests that the autoantibody against ADP / ATP carrier of heart mitochondria is one of the causes damaging myocardial function in DCM, and the detection of the autoantibody might be valuable to the diagnosis of DCM.
