二尖瓣直视成形术24例

Direct Vision Mitral valve Reconstruction.A Series of 24 Consecutive Cases

报告２４例二尖瓣直视成形术。其中１０例在成形术时应用了我们自制的软质成形瓣环。本组无手术死亡病例，无术后并发症。术后随访２月～４年９个月（平均２年），心功能均有显著改善。近、中期疗效满意。作者介绍手术适应证、手术方法及自制成形环的优点。指出二尖瓣及瓣环成形术可推广应用，如能严格掌握手术适应证、术中判断正确、处理慎重和操作仔细，均能取得良好效果。

Twenty four cases were operated on from April.1987 to Feb,1992.Fourteen were females and 10 males. Their age were 7￣61(av,32) and cardiac function, grade II in 10 and III in 14.The lesions showed rheumatic valvular stenosis in 5,non-rheumatic insufficiencies in 19.Valvuloplasty and annuloplasty were performed with various techniques according to the Pathoanatomical changes of the valve. Annulus rings made in our own institute were employed in 10 patients of this series. Postoperative course was uneventful without operative mortality.Two months to 4 years and 9 months(av.2 years) follow-up study found grade I cardiac function in all patients. There was no mitral regurgitation in 13,very mild in 9.mild to moderate in 2 but no clinical symptoms. One patient died 2 years later with a late mortality rate of 4.2%.
