Evaluation of different electrode insertion techniques in cochlear implantation

摘要 目的:评价人工耳蜗植入手术乳突切开后鼓室进路并与无乳突切开外耳道进路比较。方法:经乳突切开后鼓室进路人工耳蜗植入24例,术前常规行颞骨高分辨率CT、MRI检查,均为中耳及内耳结构正常,乳突气化良好病例。结果:人工耳蜗植入24例全部成功。1例病人术中鼓索神经切断。没有出现面神经麻痹、外耳道或鼓膜穿孔。结论:经乳突切开后鼓室进路人工耳蜗植入方便安全,疗效稳定,并发症少,应是首选术式。 Objective: To evaluate electrode insertion techniques of cochlear implantation(CI) with and without mastoidectomy. Methods: A retrospective analysis was performed to estimate the results of CI performed in our hospital from March, 2002 to October 2004 .All patients underwent preoperative examinations by using HRCT and MRI, and all had normal structure of middle and inner ear and pneumatic mastoid. Results:Full insertion of stimulation electrodes was finished in all 24 patients . One patient had tympanichord cut off intraoperatively . No patients had facial nerve paralysis, tympanic membrane, or external auditory canal perforation. Conclusion:The CI with mastoidectomy and posterior tympanotomy is safe and effective which should be the first selected method.
