肥厚性心肌病MR心肌灌注成像研究——附33例分析 被引量:8

摘要 目的探讨肥厚性心肌病MR心肌灌注成像特点和延迟强化心肌的功能表现。方法采用不同的MR成像序列,对33例肥厚性心肌病患者,行心脏形态、功能及心肌灌注成像。观察分析心肌灌注成像特点,用Argus心功能分析软件定量分析左心室整体及异常强化区心肌功能。结果24例(24/33)出现延迟期心肌强化,中层心肌强化发生率最高(22/24),强化形式为散在斑点状、不均匀片状及均匀致密片状强化。延迟强化者心肌厚度均大于无强化者,其收缩期心肌增厚率显著减低。散在斑点状强化心肌者局部心肌增厚率显著大于不均匀片状或均匀致密片状强化者。结论肥厚性心肌病MR心肌灌注成像可出现延迟期心肌强化。MR心肌灌注成像可用于检测肥厚性心肌病的心肌纤维化。 Objective To study features of hypertrophic cardialmyopathy(HCM) obtained by perfusion MRI and functions of late myocardial enhancement.Methods Varied MRI imaging sequences were used to study 33 cases with HCMin morphology, function and perfusion imaging aspects. Argus software was usedto quantitatively evaluate functions of entire left ventricle and abnormallyenhanced area. Results 24 cases (24/33) displayed late myocardial enhancement,the mid-layer was the highest (22/24). Enhancement took forms of scattered spots,uneven patches or even dense patches. Those manifesting late myocardialenhancement were thicker than those without enhancement in myocardial thickness,and were obviously low in thickening rate in systolic phase. Those manifestingspotty enhancement were significantly thicker than those manifesting unevenpatches or even dense patch in thickening rate of local myocardium. ConclusionLate myocardial enhancement may appear in MR perfusion of HCM. Theperfusion imaging can help to detect myocardial fibrosis of HCM.[
