肿瘤坏死因子受体-1在甲状腺癌中的表达及其意义

摘要 目的:研究肿瘤坏死因子受体1(tumornercosisfactorreceptors1,TNFR1)在甲状腺癌组织中的表达及其与临床及病理因素的关系。方法:应用免疫组织化学SP法和显微图像分析技术检测41例甲状腺癌中TNFR1的表达,并与9例甲状腺腺瘤及7例腺瘤旁形态学正常的甲状腺组织作对照。结果:①甲状腺癌组织中TNFR1的表达水平显著高于腺瘤组及正常甲状腺组(P<0.001),而在甲状腺腺瘤和正常甲状腺组织中的表达水平无显著性差异(P>0.05)。②在甲状腺癌各临床病理参数中,TNFR1的表达只与病理类型有关,在乳头状癌组中的表达水平高于滤泡状癌组(P<0.05);而与年龄、性别、肿瘤大小、浸润程度及淋巴结转移等因素无关(P>0.05)。结论:TNFR1参与了甲状腺癌的发生发展过程,可以作为甲状腺癌诊断与鉴别诊断的病理学指标之一,但不能作为评估甲状腺癌生物学行为和预后的有关指标。 Objective:To investigate the expression of tumor necrosis factor receptor-1 (TNFR1) in thyroid carcinoma and its relationship with clinicopathological factors.Methods:SP immunohistochemical method was used to determine the expression of TNFR1 in 41 specimens of thyroid cancer tissues,9 thyroid adenomas and 7 normal thyroid tissues adjacent to adenoma.Results:① The level of TNFR1 in thyroid carcinoma was significantly higher than that in thyroid adenomas and normal thyroid tissue (P<0.001),but there was no significant difference between thyroid adenomas and normal thyroid tissue (P>0.05).② In all clinicopathological parameters of thyroid cancer,TNFR1 and pathological types were significantly correlated:TNFR1 expression was up regulated in papillary thyroid cancer (PTC)(P<0.05),but it was not related with other factors such as sex,age,the depth of invasion,lymph node metastasis and the size of tumor (P>0.05).Conclusions:TNFR1 was thought to play a important role in the carcinogenesis of thyroid carcinoma.It would be serve as a pathological index for diagnosis and differential diagnosis of thyroid carcinoma,but it could not be regarded as an useful index for assessing the biological behaviors and prognosis of thyroid carcinoma.
