摘要
目的 观察卡介苗多糖核酸(BCG PSN)治疗变应性鼻炎的机制及其疗效。方法 将60例变应性鼻炎病人随机分为BCG PSN治疗组和息斯敏对照组,治疗前后分别采用酶联免疫吸附试验(ELISA)测定患者外周血单个核细胞(PB MCs)培养上清液中的白细胞介素 4(IL 4) ,白细胞介素 5 (IL 5 ) ,干扰素γ(IFN γ) ,粒细胞 巨噬细胞集落刺激因子(GM CSF)。同时于治疗后1周、治疗后2个月进行疗效评定。结果 病人组PBMCs上清液中IL 4、IL 5、GM CSF含量显著增高,而IFN γ含量显著减少。BCG PSN治疗后病人组上清液IL 4、IL 5、GM CSF水平明显降低,而IFN γ含量显著升高,差异有显著性(P <0 0 1)。息斯敏对照组治疗前后,上述指标变化差异无显著性。治疗2个月后疗效评定,BCG PSN治疗组显效率为66 7% ,优于息斯敏对照组(4 0 % ) (P <0 0 5 )。结论 BCG PSN能纠正变应性鼻炎病人体内Th1 和Th2 细胞因子的失平衡状态,为治疗变应性鼻炎提供了理论依据,临床疗效也表明BCG PSN治疗变应性鼻炎疗效可靠。
Objective To explore the mechanism of antiallergic action of BCG-polysaccharide nucleic acid (BCG-PSN) in allergic rhinitis. Methods Sixty patients with allergic rhinitis were randomly divided into BCG-PSN treated group and control group with Astemizole employment. IL-4,IL-5,GM-CSF,and IFN-γ levels in culture supernatant of peripheral blood mononuclear cells (PBMCs) were determined by ELISA. Results Supernatant IL-4,IL-5,GM-CSF levels were all significantly increased, while IFN-γ level was significantly decreased in these allergic rhinitis. After treatment of BCG-PSN, supernatant IL-4,IL5,GM-CSF levels were significantly reduced, and IFN-γ level was significantly increased in treated group. No significantly changes was seen in these indexes in control group. Conclusion T and B cells of allergic rhinitis are activated, so Th_1/ Th_2 cytokines are in imbalance state. BCG-PSN may inhibit Th_2 cell activation and correct the imbalance state of the Th_1 and Th_2 cytokines, by which BCG-PSN may inhibit allergic airway inflammation and control allergic rhinitis.
