摘要
我们自1991年6月至1992年6月,采用放射免疫分析法检测42例不同烧伤面积病人的血清游离T_3(FT_3)、游离T_4(FT_4)高灵敏TSH(S-TSH)。结果显示:虽然FT_3、FT_3血清含量有随烧伤面积增加而减少的趋势,但其值均在正常范围,这说明创伤病人有一定量的甲状腺素来维持最低的基础代谢,否则难以生存。与文献报道烧伤后T_3、T_4的变化不同。T_3、T_4血清含量受低蛋白(即载体蛋白浓度)下降、免疫功能低下等多种因素影响,T_3含量又随T_4在外周代谢的异常而变化。FT_3、FT_4血清含量甚微,直接作用靶细胞,能完全表达甲状腺激素功能特点,受外界影响较少,因此烧伤病人监测FT_3、FT_4准确性高,值得进一步临床观察和应用。
Using radioimmunoassay serum FT3 and FT4 and S-TSH were determined in 42 patients with different sizes of burn. The results were all in the normal range, indicat- ing that there was only sufficient amount of thyroxine in trauma patients to maintain the lowest basal metabolism for living. The changes in the total T_3 and T_4 did not conform to that reported elsewhere. It deserves further clinical observation and study.
