摘要
目的观察慢性充血性心力衰竭(CHF)患者美托洛尔(metoprolol)治疗前后QT离散度(QTd)的变化,及美托洛尔改善CHF患者预后的机制。方法将70例CHF患者随机分为常规治疗组(n=32)和美托洛尔治疗组(n=38),分别测定两组治疗前后QTd的变化,并进行对照分析。结果CHF患者给予美托洛尔治疗后,QTd较治疗前明显下降,差异有显著性;与常规治疗组相比明显减低,差异有显著性(P<0.05)。并且给予美托洛尔治疗后,心律失常发生率下降,差异有显著性。结论美托洛尔CHF患者QTd显著下降,可能是美托洛尔改善CHF患者预后的原因之一。
Objective To explore the change of QT dispersion (QTd) in patients with chronic heart failure(CHF) before and after interference by metoprolol, and the methanism by which metoprolol improvement of prognosis in patients with chronic heart failure. Methods Seventy patients with CHF were randomized into and conventional treatment group(n=32) metoprolol treatment group(n= 38), to analyse the change of QTd with two groups before and after interference. Results After interference by metoprolol, QTd was significantly decreased in patients with CHF; comparison of conventional treatmentgroup, QTd was also significantly decrased; and the rate of occurrence arrhythmia was significantly decreased .Conclusion In patients with CHF, metoprolol can reduce QTd which could be one of the reason that metoprolol can improve the prognosis in patients with CHF.
