高龄股骨粗隆间不稳定型骨折半髋成形术的临床观察

Clinical Observation on the Treatment of Elderly Patients with Unstable Intertrochanteric Femoral Fracture by Hip Hemiarthroplasty

目的评估半髋成形术治疗高龄患者不稳定型股骨粗隆间骨折的临床疗效。方法从1999～2004年我们采用骨水泥型双极假体半髋成形术治疗30例高龄股骨粗隆间骨折患者进行回顾性研究。平均年龄86岁(80～92岁),随访时间平均2a(1～3a)。结果无1例死于术后并发症,3例失随访,27例术后1周能扶助行器行走,无关节脱位或无菌性松动或髋臼磨损。Harris髋关节功能评分本组7例获得优,18例获得良,2例为可。结论对于年龄80岁以上,不稳定型粗隆间骨折的患者采用半髋成形术治疗效果良好。

Objective To evaluate the clinical results of hip hemiarthroplasty for the treatment of old patients with unstable intertrochanteric femoral fractute. Methods Between 1999 and 2004 we treated 30 elderly patients with unstable intertrochanteric fractures by primary hemiarthroplasty using a cemented bipolar prosthesis. The results of hemiarthroplasties were reviewed. Mean patient age was 86 (80～92) years and mean follow-up was 42 (12～36) months were reviewed. Results No patients died for operative complication post-operative. Three patients were lost to follow-up, Twenty-seven patients were able to walk with a walker in the first post-operative week. There were no dislocations or aseptic loosening or acetabular. We obtained excellent and 18 good and 2 fair after 12 months accordig to the Harris hip-scoring system. Conclusion Hip hemiarthroplasty for the treatment of old (>80 years) patients with unstable intertrochanteric femoral fracture had excellent clinical results.
