食管贲门癌患者围手术期血液流变学的改变及临床意义 被引量:24

SIGNIFICANCE OF PERIOPERATIVE ELEVATION OF BLOOD VISCOSITY IN PATIENTS WITH ESOPHAGEAL OR CARDIAC CARCINOMA

摘要 我们对21例食管贲门癌(研究组)开胸手术的男性患者手术前后血液流变学的变化进行了研究,并取25例健康人与研究组术前比较。结果表明:研究组血粘度高于对照组(P<0.001),术前显著低于术后即刻和术后1天(P<0.01),血粘度升高可能是食管贲门癌治疗效果差的原因之一,术后血粘度升高与心肌梗塞的发生可能有一定关系。为此,老年患者应重视围手术期处理,血粘度升高者,手术前后应采取降低血粘度的措施,尤其是术后2天内尽量少用增加血粘度的胶体液,防止血粘度过高引起的心脏并发症。 Abstract The blood viscosity of 21 patients with esophageal or cardiac carcinoma during preoperative periodwas measured and compared with that of 25 healthy persons.The results indicate that the bloodviscosity in patients is higher than that in the control grorp(P<0.001)immediately and one day afteroperation.Elevation of blood viscosity is not only related to myocardial infarction,but also may be oneof the causes for the poor therapeutic effect in patients with carcinomas.It is suggested that propermeasures should be taken to control the elevating blood viscosity in patients,especially the elderly onesduring the perioperative period.Colloid fluid which is believed to increase blood viscosity should beavoided within the first two days after operation to prevent cardiac complications.
