树突状细胞浸润、微血管密度在进展期胃癌中的预后意义 被引量:1

摘要 目的研究进展期胃癌中树突状细胞浸润、微血管密度和临床病理学特性之间的相关性及其预后意义。方法用免疫组化法通过使用图像分析仪检测50例进展期胃癌细胞中树突状细胞标志蛋白S-100的表达、微血管密度。结果50例进展期胃癌中树突状细胞均值为(10.160±7.939)/mm2。微血管密度均值(215.240±66.124)/mm2。树突状细胞浸润密度在有淋巴结转移的胃癌组低于无淋巴结转移的胃癌组(P=0.015)。有淋巴管侵犯的胃癌组中树突状细胞浸润密度低于无淋巴管侵犯的胃癌组(P=0.011)。微血管密度在有淋巴结转移组高于无淋巴结转移组(P=0.034)。50例患者5年生存率中,树突状细胞高浸润组(≥10.16/mm2)明显好于树突状细胞低浸润组(<10.16/mm2)(P=0.001)。低微血管密度组(<215/mm2)明显好于高微血管密度组(≥215/mm2)(P=0.016)。同时具有高树突状细胞浸润,低微血管密度组预后最好。结论进展期胃癌中肿瘤内低树突状细胞浸润,高微血管密度,可以促进疾病进展,可能与较差预后相关。 Objective To investigate the correlation between dendritic cell (DC) infiltration, microvessel density (MVD) and the clinicopathologic characteristics in advanced gastric carcinoma. In addition, the prognostic signiflcance of dendritic cell infiltration and microvesse1 density was evaluated. Methods The S-100 expression of marked protein of dendritic cells and microvessel density in the tumor were detected by means of image analysis system using immunohistochemical method. Results The DCs mean of the 50 tumors was ( 10.160± 7.939)/mm2. The MVD mean of the 50 tumors was ( 215.240± 66.124)/mm2. The numbers of dendritic cell infi1tration in the lymph node metastasis groups were lower than that of without lymph node metastasis groups (P= 0.015) .The DCs in gastric cancer groups with lymphangial invasion were lower than that of without lymphangial invasion. The microvessel density in the lymph node metastasis groups were higher than that of without lymph node metastasis groups (P= 0.034). In the 5-year survial rate of 50 patients,the groups who had tumors with high DCs (≥ 10.16/mm2) was significanly better than the groups who had tumors with 1ow DCs (< 10.16/mm2)(P= 0.001), the groups who had tumors with low MVD (<215/mm2) was significantly better than the groups who had tumors with high MVD (≥215/mm2)(P= 0.016). Prognosis of the groups with high DCs and low MVD was the best of all groups. Conclusion Low dendritic cel1 infiltration and high MVD may contribute to disease progression and could be correlated with a poorer prognosis.
