双侧唇裂Ⅱ期修复术：一种外鼻畸形的矫正新方法

SECONDARY REPAIR OF CONGENITAL BILATERAL CLEFT LIP:A NEW TECHNIQUE FOR CORRECTION OF NASAL TIP

对双侧唇裂术后鼻畸形矫正尚无十分满意的方法，作者参照Ｖ－Ｙ成形术原理，将前唇皮肤、皮下复合组织瓣分别分离至鼻尖部，形成２个瓣，小心保护经鼻尖垂直向下至前唇的２条小血管，使血管紧贴皮下复合组织瓣，将皮下复合组织瓣折叠３～４层，缝在鼻尖部两侧鼻翼软骨上，鼻尖部隆起效果良好，共作手术１１例，追踪现在半年至１年半，疗效较满意。本法适用于双侧唇裂Ⅱ期修复矫正鼻小柱过短和鼻尖低平的患者，使鼻、唇、颏达到和谐协调的目的。手术方法简单，易于推广应用。

ntil now , no any technique for the correction of flat nasal tip was satisfied postopera- tively and poor result was often seen with prolonging nasal column procedure alone. For this reason , the authors have adopted new technique── folding the composite prelabial flap to correct the flat nasal tip for 11 cases , and good result was observed during one and a half years follow-up. The key point for this technique is according to the principle of V-Y plasty that is undermining both the skin and subcutaneous prelabial tissue to the nasal tip with carefully protecting the two minor vessels descending from the nasal tip to the prelabium , with the two vessels attached to the composite subcutaneous flap , folding into three to four layers in the tip itself,a ideal figure will be obtained. The technique is suitable for those who need to repair their short nasal column and flat tip. The technique is not so comlplicated , therefore ,it can be widely employed.
