人工肝支持系统血浆置换疗效的相关因素分析

Analysis of related factors on efficacy of plasma-exchange therapy of artificial liver support system

[目的]探讨人工肝支持系统血浆置换疗效的影响因素.[方法]对苏州市五院2001～2003年231例实施血浆置换的患者的疗效情况进行调查.相关因素进行Logistic回归分析.[结果]以血浆置换治疗有效与否作为应变量,单因素非条件Logistic回归分析结果有显著意义的变量为置换前有肝大、腹水、浮肿、白球倒置和少尿症状,然后将以上5个因素进行逐步Logistic回归分析显示,腹水和浮肿这2个因素是影响血浆置换疗效的独立相关因素.[结论]患者在实施血浆置换前是否已经出现腹水和浮肿将影响血浆置换的疗效.

Objective] To study related factors on efficacy of plasma-exchange therapy. [Methods] History and results of 231 patients who took plasma-exchange therapy in the fifth people′s hospital of suzhou from 2001 to 2003 were collected. Related factors were examined through univariate and multivariate analyses. [Results] Taking efficacy of therapy as the dependent variable, from univariate analysis, variable that significantly associated with attributable mortality were hepatomegaly, ascites, edema, content of albumin lower than that of globulin and oliguresis before therapy.Through step-forward logistic regression analysis, ascites and edema were defined as independent significant factors influencing the results. [Conclusion] Ascites and edema before therapy were responsible for the efficacy of plasma-exchange.
