肾上腺皮质癌19例报告

Adrenocortical carcinoma:a report of 19 cases

目的探讨肾上腺皮质癌临床特点和治疗方法,提高肾上腺皮质癌的诊治水平。方法回顾性分析经病理确诊的19例肾上腺皮质癌的临床资料。肿瘤直径1～20cm,平均9.3cm,1～5cm4例,5～10cm6例,>10cm9例。Ⅰ期2例,Ⅱ期6例,Ⅲ期5例,Ⅳ期6例。结果手术完整切除13例,姑息性切除4例,仅作活检2例。术后平均生存28个月。其中5例行腹腔镜手术,4例至今生存,1例2年后死于肺转移。结论早期诊断、早期手术是治疗关键,腹腔镜手术也是治疗肾上腺皮质癌有效的方法。

Objective To study the clinical features and management of patients with adrenocortical carcinoma.Methods Clinical data of 19 cases of adrenocortical carcinoma were retrospectively analyzed.The diameter of the tumor was 1～20 cm,with an average diameter of 9.3 cm:1 to 5 cm in 4 cases,5 to 10 cm in 6 cases and more than 10 cm in 9 cases.There was 2 cases of Lee stage Ⅰ,6 of stage Ⅱ,5 of stage Ⅲ and 6 of stage Ⅳ.Results Thirteen of the 19 cases underwent complete resection and 4 incomplete resection.Only 2 cases underwent biopsy.The mean survival time was 28 months.In 5 cases subject to laparoscopic resection,4 were still survived,but 1 patient died of pulmonary metathesis 2 years later.Conclusion Laparoscopic surgery is an effective method to treat adrenocortical carcinoma.Early diagnosis and prompt surgery are important.
