疤痕子宫284例剖宫产术中出血原因分析

Clinical analysis of causes of intraoperative hemorrhage in 284 cases of cesarean section with scar uterus

目的探讨疤痕子宫剖宫产术中出血发生原因和预防措施。方法对284例疤痕子宫剖宫产和568例首次剖宫产病例导致的术中出血原因进行分析对比。结果疤痕子宫组的出血量、术中出血发生率显著高于首次剖宫产组(P<0.05)。结论疤痕子宫剖宫产术中出血多的主要原因与子宫存在瘢痕有关。首次剖宫产子宫切口术式、位置的选择和胎盘附着的部位,对再次剖宫产术中出血起决定性作用。

Objective To explore the causes and the preveution of intraoperative hemorrhage in patients with cesarean section complicated scar uterus.Methods The causes of intraoperative complication were analyzed retrospectively in 284 cases of cesarean section with scar uterus and 568 cases of primary cesarean section.Results The average blood loss and rate of intraoperative hemorrhage in scar uterus group were significantly increased compared with that of primary cesarean section group(P<0.05).Conclusion The reason with more intraoperative bleeding in the group of scar uterus is higher than the primary cesarean section.the type of previous uterine section, choose of the position and the locus of placenta play an important role in intraoperative hemorrhage of cesarean section with scar uterus.
