摘要
目的通过螺旋CT结合多平面容积重建(MPVR)、上气道仿真内窥镜三维成像,探索阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者上气道结构的功能特点。方法正常对照组7例,OSAHS组25例。每位受检者采用螺旋CT对鼻咽顶部至舌骨下缘之间的上气道,分别在平静呼吸、呼气末、吸气末和M櫣ller动作后连续扫描,测量各形态功能参数,并进行对比研究。结果OSAHS患者的上气道塌陷度较正常人明显增加,但与病情严重度则不完全相关;OSAHS患者上气道各平面矢状与冠状径的塌陷度无统计学差异(P>0.05),但在腭后区和舌后区冠状径塌陷度绝对值较矢状径增大。结论螺旋CT扫描结合MPVR重建是一种方便、高效、无创和可靠的检查方法。
Objective To investigate the upper airway characteristics of obstructive sleep apnea-hyponea syndrome (OSAHS) patients by upper airway multiplate volume reconstruction (MPVR) and virtual endoscopy (VE). Methods In 25 OSAHS patients (according to PSG) and 7 persons without habitual snoring axial CT of upper airway was performed during awake. The parameters of upper airway were measured. Results OSAHS patients showed much larger collapsibility than controls. There was no significant correlation between collapsibility and severity of OSAHS. There was also no significant difference in collapsibility between the sagittal and coronal plane in the OSASH patients. Conclusion By upper airway MPVR and virtual endoscopy, we can explore not only the shape but also the functional characteristics of OSAHS patients.
