The Dynamical Changes of Serum Albumin and Prealbumin During the Last Four Months of Elderly Patients with Dementia

【目的】探讨老年痴呆患者临终前血清白蛋白(SAL)、前蛋白(SPA)的动态变化及其临床意义。【方法】回顾性研究老年痴呆患者的死亡病例,分析死前四月内的SAL、SPA的动态检测结果,SPSS11.0 软件包统计。【结果】SAB、SPA在死前四月内明显低于正常成人水平,且呈进行性下降趋势。【结论】SAB、SPA进行性下降可能是临终前肝脏合成功能受损日渐加重甚至衰竭的结果,可视为临终前生化表现之一,亦可作为疾病预后不良的指标。

ObjectiveTo investigate the dynamical changes of serum albumin (SAL) and prealbumin (SPA) during the last four months of elderly patients with dementia.The medical record of death retrospectively reviewed according to the critera as followings: ①Dementia was the main disease without chronic impairment of kidney or liver, diabetes mellitus, tumor, connective tissue diseases, and chronic febrile disease; ②Patients were supported by rational nutrition treatment during their last half year; ③SAL and SPA were measured every month of their last four months.The levels of SAL and SPA in the observation window were lower than that of normal adults, and decreased with time.[Conclusion]The dynamical reductions of SAL and SPA may be the results of multiple organ dysfunction presenting the impairment of liver in protein synthesis, may regard as manifestations of inpending death and predictors of bad prognosis.
