老年病人颅内肿瘤术后并发呼吸衰竭的相关因素分析

ANALYSIS ON THE RELATED FACTORS OF POSTOPERATIVE RESPIRATORY FAILURE IN ELDERLY PATIENTS AFTER THE SURGERY OF INTRACRANIAL TUMOR

目的探讨老年病人颅内肿瘤术后发生呼吸衰竭的相关因素。方法回顾1993￣2003年华东医院神经外科经手术治疗的老年颅内肿瘤患者76例,分析诸多临床因素与老年病人术后发生呼吸衰竭的关系。应用SPSS11.0软件进行单因素分析和多因素Logistic回归分析。结果单因素分析显示,病人相关因素:高龄、慢性阻塞性肺病(COPD)史、术前合并肺部感染、术前肺功能减退、白蛋白水平及手术相关因素:颅内肿瘤手术部位、术后并发肺部感染,与术后呼吸衰竭的发生有关。多因素分析显示,高龄、COPD史、术前肺功能减退、颅内肿瘤手术部位是术后呼吸衰竭的主要影响因素。结论临床上可根据高龄、COPD史、术前肺功能减退、颅内肿瘤手术部位等危险因素采取相应措施,减少术后呼吸衰竭的发生。

Objective To explore the related factors associated with postoperative respiratory failure(PRF) in elderly patients after the surgery of intracranial tumor. Methods A retrospective analysis of clinical features in 76 cases of cerebromas in elderly patients,who were operated during 1993￣2003,were studied.Univariate analysis and multivariate logistic regression analysis were used to evaluate the relations between related factors and PRF. Results Multivariate analysis identified that advanced age, history of COPD,the inadequacy of preoperative lung function and intracranial tumor location were the significant factors of PRF. Conclusions Based on these related factors,it is very important in order to take necessary measures in order to decrease the development of PRF in elderly patients undergoing neurosurgical operations.
