人工晶状体植入术后瞳孔变形移位的相关因素探讨

Investigating the influence factor of pupilary deformation and shift after cataract extraction and in plantation of intraocular lens

目的探讨人工晶状体植入术后引起瞳孔变形移位的相关因素。方法对在1999年1月～2004年12月间在我院行白内障摘除及人工晶状体植入术的患者进行术后随访,观察术后瞳孔变形情况,并回顾性分析其临床资料,包括患者有无糖尿病、高度近视眼,所采用的手术方式、术中并发症及术后是否正确用药。结果本组患者术后发生瞳孔变形移位的39只眼,占20%,不同程度的伴有条索状牵引、虹膜萎缩、瞳孔缘撕裂、虹膜与人工晶状体粘连等。结论人工晶状体植入术后瞳孔变形的相关因素有:术中后囊膜破裂、虹膜损伤、人工晶状体襻未植入囊袋内、术后未及时散瞳和用足量的激素等。

Objective To investigate the influence factor of pupilary deformation and shift after cataract extraction and implantation of intraocular lens(IOL).Methods A retrospective study of 189 cases(196eyes) which received cataract extraction and IOL during Jan.1999 to Dec.2004. The information such as history of diabetes mellitus, history of high myopia eyes, the method of surgery, complication occurred in surgery, properly postoperative therapy, pupilary deformation and shift was collected and analysis by logistic regression. Results The eyes which occurred pupilary deformation and shift were 39, in 20% with cordal traction, iridolepthynsis, lacerated pupilary margin, synechia of iris and IOL, et al. Conclusion The post-operative pupilary deformation and shift are related with rupture of posterior capsule, iris injury, improper postoperative management et al.
