JSB1和C219蛋白在结肠腺癌的表达及其意义

Study on Expression and Significance of JSB1 and C219 Protein in Colon Adenocarcinoma

目的:研究P糖蛋白(P gp)的单克隆抗体JSB1和C219蛋白与结肠腺癌的耐药、生物学行为及预后的关系。方法:用免疫组化S P法检测32例正常结肠黏膜、20例结肠腺瘤、96例结肠腺癌、30例死胎结肠黏膜组织中JSB1和C219蛋白的表达。结果:JSB1在正常结肠黏膜、结肠腺瘤、结肠腺癌及死胎结肠黏膜表达的阳性率显著高于C219(P<0.01)。JSB1和C219在死胎结肠黏膜的表达均比正常结肠黏膜高,其中前者有统计学意义(P<0.01)。JSB1和C219在结肠腺癌的表达均比正常结肠黏膜高,其中后者有统计学意义(P<0.05)。二者表达阳性率分别为57.29%(5596)、33.33%(3296)与结肠腺癌的组织学类型、浸润程度、淋巴结转移及Dukes分期无明显相关性(P>0.05)。单独表达JSB1或C219、无阳性表达的结肠腺癌病例5年存活率均显著高于二者均呈阳性表达的病例(P<0.01)。结论:结肠腺癌由P gp介导的多药耐药性是天然性的,并且是由多种P gp异构体共同形成的。检测JSB1和C219的表达可作为指导结肠腺癌化疗用药和判断预后的参考指标。

Objective:To study the relationship between the expression of JSB1,C219 monocolonal antibody of P-glycoprotein(P-gp)and drug-resistance,biologic behavior,prognosis of colon carcinoma.Methods:The expression of significant higher than that of normal colon mucosa(P<0.01).C219 expression rate of colon carcinoma was significant higher than that of colon adenoma(P<0.05).The expression rate of JSB1 was significant higher than that of C219 in colon carcinoma,normal colon mucosa,colon adenoma and colon mucosa of foetus(P<0.01). The level of JSB1 and C219 expression were not JSB1 and C219 were examined by immunohistochemical technique(S-P method)in 96 cases of colon carcinoma who had not received chemotherapy or radiotherapy,32 cases of normal colon mucosa,20 cases of colon adenoma and 30 cases of colon mucosa of foetu. Results:Positive expression of JSB1 and C219 were detected in 55 and 32 of 96 cases with colon carcinoma,respectively. JSB1 expression rate of colon mucosa of foetus was associated with histologic grade,depth of invasion of the bowel wall,lymph node metastases or Dukes' stage in colon carcinoma(P>0.05).The 5-year survival rate in cases without positive expression and only JSB1 or C219 was significant higher than that in cases with positive expression of JSB1 and C219(P<0.01).Conclusion:The multi-drug resistance with P-gp of colon carcinomatous cell is natural.Expression of JSB1 and C219 monocolonal antibody of P-gp in carcinomatous tissue has an instructive significance for chemotherapy and prognosis of colon carcinoma.
