以脾作参照应用T_1加权像诊断脂肪肝TR值的选择及应用价值的探讨

摘要 目的:以脾作为参照物,通过选择适当的TR值,进一步探讨T1加权像对脂肪肝的诊断价值。方法:对5 0例正常人和5 2例脂肪肝患者(经CT检查确诊)全部进行TR值不同的两种T1加权肝脏横断面成像。对两种T1加权像中正常组和脂肪肝组的肝-脾信号强度差别分别进行对比分析。结果:以肝脏信号强度高于脾信号强度作为诊断脂肪肝的标准,较长TRT1加权像(TR/TE =6 80mc/ 2 5ms)中,正常组有4 8例(96 % )显示肝-脾信号强度基本相等,仅2例(4% )显示肝脏信号强度略高于脾信号强度。脂肪肝组全部显示肝脏信号强度高于脾信号强度,两者比较有极显著性差异(P <0 0 1)。结论:以脾作为参照物,适当选择TR值,T1加权像是诊断脂肪肝的有效方法。应用低场磁共振推广此方法具有实用价值。 Objective:To explore the value of “T_1 Weighted Imaging' in diagnosis of fatty liver by selecting a proper TR value.Methods:2 kinds of “T_1 Weighted Imaging' with different TR values were carried out in 50 healthy people and 52 patients with fatty liver (diagnosed by CT).Then respectively contrast and analyze the strength difference of liver and spleen signals of the normal group and the fatty liver group in the 2 kinds of “T_1 Weighted Imaging'.Results:Take the strength of liver signals higher than that of spleen signals as the standard of diagnosing fatty liver.In the “T_1 Weighted Imaging' (TR/TE=680ms/25ms) with longer TR,48 cases (96 percent ) out of the normal group shows a basically equal strength of liver and spleen signals while 2 cases (4 percent) show the strength of liver signals slightly higher than that of spleen signals.Each case in the fatty liver group shows the strength of liver signals higher than that of spleen signsls,there is extremely marked difference (P<0.01) in contrasting the two groups.Conclusion:Taking spleen as a reference,by selecting a proper TR value,“T_1 Weighted Imaging' is a good method to doagnose fatty lover.This method is of important popularizing value to the magnetic resonance with low intensity of field.
