经尿道前列腺电气化治疗晚期前列腺肿瘤所致尿道梗阻

Transurethral Electrovaporization of the Prostate in the Treatment of Advanced Prostatic Neoplasms

目的 探讨经尿道前列腺电气化治疗、晚期前列腺肿瘤所致后尿道梗阻。方法 分析 15例确诊为晚期前列腺肿瘤的患者 ,其中前列腺癌 14例 ,前列腺平滑肌肉瘤 1例。骨转移 8例。 10例术前因尿潴留行导尿或耻骨上膀胱造瘘。同时采用经尿道前列腺电气化术 (TVP)手术结合内分泌治疗。结果 15例临床症状、最大尿流率、剩余尿量均明显改善。血PSA由术前平均 78.39ng/ml降至 2 .8ng/ml。骨痛患者症状减轻或消失。骨转移灶稳定。

Objective To evaluate transurethral electrovaporization of the prostate (TVP) in the treatment of bladder outlet obstruction caused by advanced prostatic neoplasms.Methods Fifteen patients with advanced prostatic neoplasms were treated by endocrine therapy and TVP,14 of whom suffering from prostatic cancer and one case suffering from prostatic sarcoma.There were 10 cases urinary retention was treated by urethral catheterization or suprapubic cystostomy.Bone scintigraphy with technetium-99m methylene diphosphonate (MDP) demonstrated multiple sites of metastatic disease was in 8 cases.Results Improvements in subjective symtoms,objective signs,maxmum flow rate and residual urine were significant.Mean PSA decreased from 78.29ng/ml preoperatively to 2.8ng/ml 3 months after the treatment.Bone pain in 6 cases was ameliorated.Bone of metastatic diseases was mot found in the X-ray examination.Conclusions TVP is considered to be safe and effective in relieving the urethral obstruction caused by advanced prostatic neoplasms so as to improve the life quality of the patients.
