摘要
目的 :该实验旨在评价小剂量多巴酚丁胺超声心动图激发试验 (Dobu -UCG)定量检测实验犬冬眠心肌的价值。建立犬心肌梗塞模型 11条 ,3- 6mon后对 7条成活的犬进行研究 ,结果与病理检查比较。结果 :Dobu -UCG后心肌梗塞区 (AMI) /非梗塞区心内膜运动幅度 (ANMI)比值 (AMI/ANMI) ,以及试验后AMI/ANMI与试验前AMI/ANMI的差值 (D)分别与四唑氮蓝染色后组织切片的冬眠心肌面积 /梗塞区总面积的比值 (ATAM/ATIM)进行对比分析 ,显示两者均高相关 (r=0 83- 0 ,90 ,P分别为p <0 0 5和p <0 0 0 1)。结论 :测量Dobu -UCG激发试验后心肌梗塞区室壁运动幅度 /非心肌梗塞区室壁运动幅度AMI/ANMI,以及试验后、前AMI/ANMI差值 (D)是定量梗塞区内冬眠心肌的较好方法 ;Dobu -UCG激发试验前心肌梗塞区室壁运动幅度不能判断梗塞区内是否有冬眠心肌以及冬眠心肌量的多少。
The purpose of this study was evaluated the value of quantitative measurement of experimental dog′s hibemating myocardium by excitation trial with low dose dobutamine echocardiography(Dobu-UCG).11 dogs with experimental myocardium infarction were bilt,3-6 monthes later,7 alive doys from 11 were give Dobu-UCG experimental study.Results:Compared with pathological examination which was ration(ATAM/ATIM) of area of total alive myocardium (ATAM) to area of total infarction myocardium(ATIM),and Dobu-UCG the test displayed that there were ration(AMI/ANMI)of wall motion amplitude of areas with myocardium infarction(AMI) to wall motion amplititude of areas with non myocardium infarction(ANMI)after Dobu-UCG,and the difference(D)between AMI/ANMI ratio before trial and after trial.The excellent correlation between AMI/ANMI ration after Dobu-UCG test and ATAM/ATIM ratio measured by histologically(r=0 83,p<0 05).The difference(D)between AMI/ANMI ratio before treal and after trial by Dobu-UCG was well correlation with that by histologically(r=0 90,P<0 001).Conclusion:(1)AMI/ANMI and D can quantify the hibernating myocardium from infarction myocardium after excitation trial by Dobu-UCG.(2)The wall motion amplititude of infarction myocardium before excitation trial by Dobu-UCG can′t dicide how many there have the areas of hibernating myocardium among infarction myocardium.
