婴儿巨细胞病毒肝炎血清白细胞介素12、白细胞介素18及肿瘤坏死因子α测定与意义

目的测定婴儿巨细胞病毒(CMV)肝炎治疗前后血清白细胞介素12(IL-12)、白细胞介素18(IL-18)及肿瘤坏死因子α(TNF-α)水平。方法采用酶联免疫吸附法检测36例CMV肝炎患儿治疗前后及21例健康婴儿血清IL1-2、IL-18及TNF-α的水平。结果CMV肝炎患儿血清中IL-12、IL-18及TNF-α的水平明显高于正常对照组(P<0.001),且均与血清丙氨酸转氨酶(ALT)显著正相关(P<0.01);治疗有效者在治疗结束后血清IL-12、IL-18和TNF-α水平均较治疗前显著降低(P<0.001),而无效者IL-12、I-L18和TNF-α治疗前后无明显变化(P>0.05)。结论血清IL-12、IL-18及TNF-α的检测可作为婴儿CMV肝炎炎症活动性和肝损害程度的评判指标,并对预测药物抗病毒疗效有重要的指导价值。

Objective To study the variation of serum interleukin -12(IL-12), interleukin-18(IL-18) and tumor necrosis factor-alpha(TNF-α) in infants with cytomegalovirus(CMV) hepatitis before and after treatment. Methods The levels of serum IL-12, IL-18 and TNF-α in 36 in fants with CMV-hepatitis before and after treatment and in 21 normal infants we re detected by enzyme-linked immunosorbent assay. Results Serum levels of IL-12, IL-18 and TNF-α in infants with CMV-hepatitis were obviously higher than those in normal infants(P< 0. 001) and had a positive correlation with alanine aminotransferase(ALT) respect ively(P< 0.01). Serum levels of IL-12, IL-18 and TNF-α markedly decr eased in responsive patients(P< 0.001) while no change was observed in pa tients with no response after treatment(P> 0.05). Conclusion In the infants with CMV-hepatitis, the levels of serum IL-12, IL-18 and TNF-α are assisted markers for estimating activity of i nflammation and degree of liver damage and have great values in predicting e ffects of treatments.
