摘要
目的研究survivin和caspase3、Ki67在肝细胞肝癌(HCC)及癌旁肝组织中的表达及其意义。方法利用RNA提取试剂获取新鲜肝癌组织RNA,用RT-PCR方法测定survivin基因mRNA;用免疫组化方法检测肝细胞肝癌及其癌旁组织中caspase3和Ki67蛋白的表达。结果17例新鲜HCC组织,survivin基因表达阳性13例,阳性率为76.5%,癌旁肝组织和正常肝组织survivin表达全为阴性。13例survivin表达阳性的肝癌组织其caspase3蛋白表达平均染色强度为0.98±0.35.显著低于4例survivin表达阴性的HCC组织1.36±0.47(P<0.01),而survivin表达阳性的HCC组织其Ki67表达强度平均评分为2.787±0.71,又显著高于survivin表达阴性的HCC组织1.662±0.38(P<0.01)。88例肝癌组织caspase3蛋白的阳性表达率明显低于癌旁肝组织(18.5%vs38.6%,P%0.01),caspase3蛋白的表达强度与HCC的分化程度相关,分化愈差,caspase3蛋白表达愈弱(F=8.651,P<0.01);Ki67在肝癌组织中其平均表达强度评分为2.44±1.12,在癌旁肝组织中为散在灶性表达,Ki67表达强度评分与HCC分化程度有关,分化愈差,Ki67表达强度评分愈高(F=9.121,P<0.01)。结论本文结果提示在HCC发生过程中survivin的表达或许可通过抑制caspase3而抑制细胞凋亡,促使细胞增殖,使HCC呈现无限制的生长及恶性程度不断增高。
Objective To investigate the expressions and clinical significances of surviving, caspase 3 and Ki67 in hepatocellular carcinomas(HCC) and pericarcinomatous liver tissue. Methods Total RNA was extracted from fresh specimens of HCC and pericarcinomatous liver tissue. Survivin mRNA amplification was performed by reverse transcription polymerase chain reaction (RT-PCR).The expressions of caspase 3 and Ki67 were detected by immunohistochemistry in hepatocellular carcinomas and pericarcinomatous liver tissues in formalin-fixed,paraffin-embedded specimens. Results RT-PCRs were performed in 17 HCC, 18 pericarcinomatous liver tissues and 4 normal liver tissue, 13 HCC specimens showed 344 bps surviving DNA band in 1 % agarose electrophoresis.but none of pericarcinomatous liver tissues and normal liver tissue showed this positive band. The positive rate of survivin in HCC was 76. 5% (13/17) The expression strength of caspase 3 of 13 survivin positive HCC specimens was 0. 98 ± 0. 35 .which was significantly lower than that of 4 survivin negative HCC specimens(1. 36 ±0. 47)(P<0. 01) ; the scale of Ki67 in 13 survivin positive HCC specimens was 2.787 ± 0.71, which was significantly higher than that of 4 survivin negative HCC specimens(1. 662 ?0.38) (P<0.01). The positive rate of caspase 3 in 88 cases HCC was significantly lower than that in pericarcinomatous liver tissue(18. 5% vs 38. 6% ,P<0. 01) ,the expression strength of caspase 3 protein in HCC was remarkably related to differentiation degree of HCC, the poorer differentiation,the weaker the expression of caspase 3 protein(F= 8. 651 ,P <0. 01). The mean scale of Ki67 in 88 cases HCC was 2. 44 ?1.12, the scale of Ki67 in HCC was remarkably related to differentiation degree of HCC, the poorer differentiation, the higher of the scale of Ki67(F= 9. 121 ,P<0, 01). Conclusion The expression of survivin maybe make uncontrolled growth and unceasingly increased malignant degree of HCC by accelerating cell proliferation and inhibiting caspase 3 resulting in apoptosis.
