硬膜外在全麻单肺通气手术中的应用

摘要 目的探索全麻单肺通气期间硬膜外麻醉的合理应用方法。方法将30例ASAⅠ~Ⅱ食道癌行根治术者,均采用全麻复合硬膜外麻醉,硬膜外采用小剂量分次给药,根据手术进展追加麻醉药,保持适宜的呼气末二氧化碳分压、气道峰压、中心静脉压,合理应用升压药;观察单肺通气期间的血压、脉搏、血氧饱和度及用药效果。结果7例在单肺通气期间出现不同程度的低血压并伴随脉搏血氧饱和度下降,经给予多巴胺后均短时间内恢复正常。结论合适的硬膜外给药时机、阻滞平面及范围,尽量避免一些容易引起肺内分流的因素,选择正确的方法处理低血压和低氧血症,硬膜外麻醉可安全有效地应用于全麻单肺通气手术中。
