摘要
目的探讨P16和cyclinE蛋白在食管鳞状上皮、增生上皮和癌变上皮中表达状况及其与鳞状细胞癌发生、发展和转移的相关性。方法采用SP免疫组织化学方法,检测食管鳞癌72例,粘膜上皮增生21例,正常对照13例和淋巴结转移癌15例中P16和cyclinE基因蛋白的表达。结果P16在正常上皮组无阳性表达,增生组为23.81%,鳞癌组为38.89%;高分化鳞癌组阳性表达率(41.67%)显著高于低分化鳞癌组(27.27%)(P<0.05);有淋巴结转移组(16.67%)和无淋巴结转移组(42.85%)比较具有显著性差异(P<0.05);原发癌组阳性率显著高于淋巴结转移癌组(P<0.05)。cyclinE在正常上皮组无阳性表达,增生上皮组为33.3%,鳞癌组为51.4%;高分化鳞癌组阳性表达率(44.44%)显著低于低分化鳞癌组(81.81%)(P<0.05);淋巴结转移组cyclinE阳性表达率(79.17%)和淋巴结无转移组(48.57%)比较有显著性差异(P<0.05)。P16和cyclinE表达与肿瘤部位和浸润深度相关性不明显。结论在食管癌中P16和cyclinE基因蛋白的表达与病理分化程度有关;检测P16和cyclinE对食管癌早期诊断有指导作用。
Objective To explore the expression and correlation of p16、cyclinE protein in esophageal epithelium,hyperplastic epithelium ,change of carcinoma epithelium and the pathogenesis ,progression, metastasis of squamous cell carcinoma. Methods Expression of p16、cyclinE protein were examined in 72 cases of esophageal squamous cancer、hyperplastic epithelium 21 cases、normal squamous epithelium 13 cases and node metastatic cancer 15 cases. Results The positive rates of p16 protein were 38.89% ,23.81% in esophageal cancer and hyperplasia epithelium. The normal epithelium group was not found of the positive expression of p16 . The positive rate in high differentiation group(41.67%) was significantly higher than that in low differentiation group(27.27%) (P<0.05 ). The esophageal cancer with local lymph node metastasis (16.67%) and the esophageal cancer without local lymph node matastasis (42.85%) were compared with statistics meaning (P<0.05). The rates of cyclinE expression in esophageal cancer, hyperplasia epithelium, normal epithelium were 51.38%, 33.33% ,0%. The high(44.44%) and low (81.81%)differentiation cancer were significant different (P<0.05). The expression rate of P16 in lymph node metastatic cancer was significenthy lower than that in primary carcinoma (P<0.05).The cyclinE expression in lymph node metastasis of esophageal cancer (79.17%) was siginificantly higher than that in without lymph node metastasis of esophageal cancer (48.57%) (P<0.05). The expressions of p16、cyclinE were independent of the location of cancer and the depth of tumor invasion.Conclusion The expressions of p16 and cyclinE in esophageal carcinoma are related with pathologic differentiation degree. To examine the expressions of P16 and cyclinE have guidance function for early diagnose in esophageal squamous cancer.
