血尿酸与2型糖尿病患者颈动脉粥样硬化相关性分析 被引量:5

摘要 目的: 探讨2型糖尿病(type2diabetesmellitus,T2DM)患者血清尿酸水平与颈动脉粥样硬化是否有关.方法:T2DM患者318例,按是否有颈动脉粥样硬化分为2组,对患者尿酸、年龄、体质量指数(BMI)、血压、肌酐、空腹血糖、血清胆固醇、三酰甘油、白细胞多重动脉粥样硬化危险因素进行多变量分析.结果: Logistic回归分析表明T2DM合并动脉粥样硬化组血清尿酸水平明显升高(P<0. 001 ),其与年龄(P=0. 001 ),收缩压(P<0 .05 )是T2DM患者颈动脉粥样硬化的危险因素,但尿酸差异经血肌酐修正后相关性消失.尿酸等影响因素的多元逐步回归分析结果显示,尿酸与血肌酐(P<0.. 001 ),颈动脉粥样硬化(P<0 .001 ),BMI(P=0 .004 ),白细胞(P=0 .001 )具有相关性.结论: T2DM患者颈动脉粥样硬化时血清尿酸水平升高,这种变化可能与肾脏对尿酸清除下降有关. AIM: To determine whether uric acid concentration is an independent factor associated with carotid atherosclerosis in type 2 diabetes mellitus (T2DM).METHODS: A cross-sectional study of 318 T2DM patients with or without carotid atherosclerosis was conducted.Uric acid and other T2DM risk factors,including age,T2DM duration,body-mass index (BMI),free blood glucose (FBG),systolic blood pressure (SBP),diastolic blood pressure (DBP),serum cholesterol (TC),triglyceride (TG),and blood white cells (WBCs),were analyzed.RESULTS: In Logistic regression analysis of carotid atherosclerosis,uric acid (P<0.001) was higher in carotid atherosclerosis group.Uric acid,together with age(P=0.001) and SBP (P<0.05),was associated with carotid atherosclerosis in T2DM.After adjustment of kidney function by uric acid /creatinine ratio,the difference disappeared.In multiple regression analysis of uric acid in all the groups,creatinine (P<0.001),carotid atherosclerosis(P<0.001),BMI(P=0.004) and WBCs(P=0.001) were independent determinants.CONCLUSION: Uric acid is an independent factor in T2DM with carotid atherosclerosis.The increased level may be associated with the injury of kidney function.
