不良固定修复体与局限性牙周炎关系探讨

The relationship between the ill fitting fixed denture and the locali zed periodontitis

目的:探讨不良固定修复体与局限性牙周炎之间的关系。方法:选择口内有固定修复体同时伴局限性牙龈炎或牙周炎的患者186例,575颗牙。分别进行牙周状况和修复体检查如修复体边缘适合性、轴面外形、邻接关系及桥体龈面与牙槽嵴的关系,对检查结果进行统计、分析。结果:186例局限性牙周炎的患者,其口内相应部位的修复体都存在修复缺陷。以修复体边缘不适合为主,占84.00%,其次是桥体龈面形态不良、邻接关系不良、轴面外形不良。结论:不良固定修复体是引起或加重局部牙周组织损伤的主要原因之一。

Objective: Analyze the relationship between the ill fitting fixed denture and the localized periodontitis. Methods: Selected 575 teeth from 186 p atients who have fixed denture restoration and localized gingivitis or periodon titis. Exam their periodontal status: oral hygiene, gingival index(GI), communit y periodontal index(CGI), radiographic and denture examination such as the margi nal fitness, axial surface shape, proximal contact and the adjacence of gingival surface of the bridge pontic and the alveolar ridge. Results: In the 186 patien ts have localized periodontitis, the corresponding area of all the restorations has some unfitness. Marginal ill fitness took up 80%, followed with the ill fitn ess of the gingival surface of the pontic, bad proximal contact and axial surfac e shape. Conclusion: The ill fitness of fixed denture is the important factor wh ich can cause or aggravate the periodontal trauma.
