X线诊断先天性肺囊性腺瘤样畸形

X-ray diagnosis in congenital cystic adenomatoid malformation of lung

目的 观察先天性肺囊性腺瘤样畸形的胸部X线表现及其诊断价值。方法 17 例先天性肺囊性腺瘤样畸形患儿,均经胸部X线检查,其中13例经手术病理证实。结果 本病胸部X线平片有三种表现:①“气胸样”改变: 9 例为此种表现,X线表现为一侧肺明显过度充气,张力很大,其中未见肺纹理结构,很象气胸表现,其中有不规则的分隔征象6 例伴纵隔肺疝;②多囊状改变: 3例表现为病侧肺呈多发囊状改变,纵隔移位明显;③肿块样改变: 1 例为肿块样改变,病变累及肺叶呈实变样改变,类似肿块影,边界整齐,其中可见不规则小囊状透光区。结论 儿童患者胸部X线表现对本病的诊断有提示作用。

Objective To evaluate the chest radiographic appearances and their diagnostic values of congenital cystic adenoid malformation of lung (CCAM). Methods Among 17 patients with CCAMs, 13 cases were confirmed by operation and pathology, and the other 4 cases were outhospitalized because of denying operation. All of 13 cases performed X-ray examination. Results Three kinds of X-ray appearances were detected in the chest radiology. Pneumothoracic-like changes for 9 cases, 6 showed unilateral lung hyperinflation, high tension, lack of lung marking combined with irregular septum and mediastinal herniation. Multiple cyst changes for 3 cases. The lung showed multiple cystic changes with mediastinum shifted to the contralateral lung. Mass-like change for 1 case. The lung showed consolidation which involve one lobe with clear margin and irregular low attenuation. Conclusion Chest radiography is a valuable method to diagnose CCAM.
