流行性腮腺炎的超声诊断及鉴别诊断

Ultrasonographic Diagnosis and Differential Diagnosis of Epidemic Parotitis

目的探讨流行性腮腺炎在超声诊断及鉴别诊断中的声像图特征。方法对30例腮腺肿大声像图回顾性分析,分析内容包括腮腺及颌下腺的大小、形态、内部回声、周围有无肿大淋巴结。结果流行性腮腺炎占全部病例的80%,声像图特征:急性期腮腺增大、边界欠清晰,其内见散在低回声,随病情好转低回声减少或消失;化脓性腮腺炎多为单侧肿大,脓肿形成期见不规则液性区或蜂窝状小腔。结论流行性腮腺炎具有特征性声像图表现,超声能为临床提供较客观的诊断及鉴别诊断依据。

Objective To investigate the features in the diagnosis and differential diagnosis of epidemic paratitis using ultrasonography(USG).Methods We retrospectively analysed the parolid ultrasonograms in 30 patients who had swelled parolid glands.The size,the contour,the internal echoes of the parolid the submadibular glands and the accompanied lymph nodes were analysed.Results The epidemic parotitis counted 80%(24/30).In the acute stage of the disease,the gland enlarged,with an indeterminate boundary and some inner scattered lower eehoes.The lower echoes were decreased or even disappeared along with the improvement of the parotitis.In purulent parotitis,only unilateral gland swelled with irregular liquid dark area or cellular small cavities different from the sonographic appearance in the epidemic.Conclusions Epidemic parotitis has its special sonographic manifestations Ultrasonography can provide objective bases for clinical diagnosis of epidemic parotitis.
