尿道记忆合金支架在骶髓损伤后排尿功能障碍中的应用

Implantion of urethral stents in sacral-injured patients with voiding dysfunction

目的介绍尿道记忆合金支架治疗骶髓损伤后排尿功能障碍的方法,并评价其疗效。方法对3例骶髓损伤后排尿功能障碍的男性患者,应用尿道记忆合金支架置入于尿道外括约肌附近,并配合Crede手法挤压下腹部排尿,运用残余尿测定、膀胱容量及肾功能的改变和B超检查等指标评估其疗效。结果3例患者置入支架后,运用Crede手法均能顺利挤压排尿,残余尿量明显下降。最长随访20个月,患者均无尿路感染、膀胱结石的发生,肾功能均正常。结论尿道记忆合金支架置入术治疗骶髓损伤后排尿困难疗效确切,能明显地提高患者生活质量,降低并发症的发生。

Objective To introduce an implantation of urethral stents to sacral-injured patiens with voiding dysfrnceion and evaluate its outcome.Methods Three sacral-injured male patients with voiding dysfunction were treated by implantation of urethral stents which were implanted nearby the external sphincter. The voiding was relied on the Credes maneuver. The following objective parameters were used to evaluate the outcome: residue volume、renal function and B-Ulthsound.Results After implantation, three patients could voide with Credes, maneuver and the residue volume decreased significantly. The longest follow-up was 36 months. There were no urinary tract infection and bladder calculus happened and all the renal functions were normal.Conclusion This procedure is exactly effcetive to the sacral-injured patient. It can drmaticly improve the life quality of the patient and decrease the complications.[
