内窥镜鼻窦手术420例临床分析

The analysis of 420 cases of endoscopic sinus surgery

目的探讨内窥镜手术治疗慢性鼻窦炎的疗效及影响手术疗效的相关因素。方法对1998～2003年间实施内窥镜手术并完成随访的420例慢性鼻窦炎、鼻息肉患者进行临床分析,420例中1型132例(31.5%),2型193例(45.9%),3型95例(22.6%)。420例全部在局麻下完成。术后随访6～36个月,平均13个月。同时对术腔及中鼻甲处理、钩突异常等因素进行讨论。结果1型治愈率为93.7%,2型治愈率为81.6%,3型治愈率为68.4%。三型之间治愈率差异有显著意义(P<0.01)。结论内窥镜手术是治疗慢性鼻窦炎、鼻息肉的有效方法。其发病与中鼻甲及钩突结构异常密切相关。手术疗效与临床分型相关。

Objective To evaluate the effect of endoscopic sinus surgery(ESS) for chronic sinusitis and the related factors which affect the effecty of surgery.Methods Four hundred and twenty cases with chronic sinusitis and nasal polyps undergone ESS from 1998 to 2003 were appraised with the standard criteria(Chinese ENT Association,1997).There are 132 cases(31.5%) in type 1,193 cases(45.9%) in type 2 and 95 cases(22.6%) in type 3.Four hundred and twenty cases underwent ESS under intranasal local anesthesia and sedation.Long-term follow-up was performed at 6 to 36 months(mean=13 months).Results The results showed that cure rate was 93.7% for type 1,81.6% for type 2 and 68.4% for type 3.There was significant difference in cure rate among different types(P<0.01).Conclusion ESS is efficacy and safe in the treatment of nasal polyps and chronic sinusitis.The cure result is directly related to the types of the disease.
